Evaluation of an antibiotic restriction policy using a special drug request form.

At a university teaching hospital, a study was undertaken to assess the frequency and appropriateness of restricted antibiotic use using a special drug request (SDR) form to collect the data. The usefulness of the SDR form itself was also assessed for its ability to provide valuable information about patient care and cost effectiveness. The results of this study are presented below.
