To investigate the relationship of the detachment of posterior hyaloid membrane and idiopathic macular hole by optical coherence tomography (OCT).
Fundus examination after dilatation of pupil and OCT were performed on 223 eyes of 193 patients with idiopathic macular hole. All macular holes were graded according to the classification proposed by Gass.
According to Gass classification, there were 21 eyes of stage I macular hole, 74 eyes of stage II, 97 eyes of stage III and 31 eyes of stage IV. OCT showed the detachment of posterior hyaloid membrane in 15 eyes (71.4%) in stage I, 38 eyes (51.4%) in stage II and 46 eyes (47.4%) in stage III. There were three kinds of imaging characteristics: (1) Perifoveal detachment of posterior hyaloid membrane (hyporeflective band) with a cyst (hyporeflective cavity) in the center of fovea and the elevating or flattening of fovea by the traction of posterior hyaloid. (2) The posterior hyaloid (hyporeflective band) adhered to the partially detached operculum (hyporeflective band) that was pulled forward. (3) The complete detachment of posterior hyaloid from the retina was seen. In some cases, the OCT image showed the development of macular hole from stage I to stage III. A scan along the papillomacular axis showed that the posterior vitreous detachment was dome shaped with the adhesion to fovea and disc.
The occurrence of posterior hyaloid detachment at the fovea is in accordance with the formation of macular hole.
采用光学相干断层扫描(OCT)研究后玻璃体脱离与特发性黄斑裂孔的关系。
对193例特发性黄斑裂孔患者的223只眼进行散瞳后眼底检查及OCT检查。所有黄斑裂孔均按照Gass提出的分类法进行分级。
根据Gass分类法,I期黄斑裂孔21只眼,II期74只眼,III期97只眼,IV期31只眼。OCT显示,I期15只眼(71.4%)存在后玻璃体脱离,II期38只眼(51.4%),III期46只眼(47.4%)。有三种影像学特征:(1)后玻璃体膜(低反射带)在黄斑中心凹周围脱离,中心凹有囊肿(低反射腔),后玻璃体牵拉使中心凹抬高或变平。(2)后玻璃体(低反射带)与部分脱离并向前牵拉的盖瓣(低反射带)粘连。(3)可见后玻璃体与视网膜完全脱离。在某些病例中,OCT图像显示黄斑裂孔从I期发展到III期。沿视乳头黄斑轴扫描显示,玻璃体后脱离呈穹窿状,与中心凹和视盘粘连。
黄斑区后玻璃体脱离的发生与黄斑裂孔的形成一致。
