To investigate the correlation between serum thyroglobulin (Tg) and thyroid stimulating hormone (TSH) in populations with non-toxic goiter.
Thyroid ultrasonography was conducted, and determination of serum triiodothyronine (FT3), thyroxin (FT4), Tg, thyroid peroxidase antibody (TPOAb), thyroglobulin antibody (TGAb) and TSH, and urine iodine were carried out among 609 persons with an average age of 35.4 in Panshan county, a mild iodine-deficient area in Liaoning province, and 1136 persons aged 39.1 on average in Zhangwu county, an iodine sufficient area in Liaoning province, totally 2 320 persons.
The serum TSH levels of the populations with diffuse and nodular goiter in Panshan county were (0.11 +/- 0.08) mU/L and (0.90 +/- 0.10) mU/L, significantly higher than those of the normal population in the same areas (1.33 +/- 0.04) mU/L, both P < 0.01. The serum TSH levels of the populations with diffuse and nodular goiter in Zhangwu county were (1.29 +/- 0.13) mU/L and (0.89 +/- 0.14) mU/L, significantly higher than those of the normal population in the same areas (1.74 +/- 0.04) mU/L, both P < 0.01. The serum TSH level of normal population in iodine deficient area was significantly lower than that of normal population in iodine sufficient area (P < 0.01). The serum TSH level of the normal population in Panshan County was significantly lower than that of the normal population in Zhangwu County (P < 0.01). The serum Tg level of the populations with diffuse and nodular goiter in Panshan county were (12.0 +/- 1.21) ng/L and (50.1 +/- 12.20) ng/L, significantly higher than that of the normal population in the same area (9.1 +/- 2.44) ng/L, P < 0.05 and P < 0.01. The serum Tg level of the populations with diffuse and nodular goiter in Zhangwu county were (12.4 +/- 2.11) ng/L and (61.2 +/- 11.86) ng/L, significantly higher than that of the normal population in the same area (7.7 +/- 2.3) ng/L, P < 0.05 and P < 0.01. The Tg level of the normal population in Panshan County was significantly higher than that of the normal population in Zhangwu county (P < 0.01). The Tg level was positively correlated with the thyroid volume in the nodular goiter population.
The occurrence of goiter is not directly related to TSH stimulation in mild iodine deficient area. Nontoxic goiter shows a thyroid autonomic function. The major cause of high serum Tg in nodular goiter may be the leakage of Tg into blood circulation caused by degeneration of large colloid follicles and destruction of follicular architecture, not the volume increase of thyroid and its autonomic function.
探讨非毒性甲状腺肿人群血清甲状腺球蛋白(Tg)与促甲状腺激素(TSH)之间的相关性。
对辽宁省轻度碘缺乏地区盘山县平均年龄35.4岁的609人以及辽宁省碘充足地区彰武县平均年龄39.1岁的1136人进行甲状腺超声检查,并测定血清三碘甲状腺原氨酸(FT3)、甲状腺素(FT4)、Tg、甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TGAb)、TSH以及尿碘,共2320人。
盘山县弥漫性和结节性甲状腺肿人群的血清TSH水平分别为(0.11±0.08)mU/L和(0.90±0.10)mU/L,显著高于同地区正常人群的(1.33±0.04)mU/L,P均<0.01。彰武县弥漫性和结节性甲状腺肿人群的血清TSH水平分别为(1.29±0.13)mU/L和(0.89±0.14)mU/L,显著高于同地区正常人群的(1.74±0.04)mU/L,P均<0.01。碘缺乏地区正常人群的血清TSH水平显著低于碘充足地区正常人群(P<0.01)。盘山县正常人群的血清TSH水平显著低于彰武县正常人群(P<0.01)。盘山县弥漫性和结节性甲状腺肿人群的血清Tg水平分别为(12.0±1.21)ng/L和(50.1±12.20)ng/L,显著高于同地区正常人群的(9.1±2.44)ng/L,P<0.05和P<0.01。彰武县弥漫性和结节性甲状腺肿人群的血清Tg水平分别为(12.4±2.11)ng/L和(61.2±11.86)ng/L,显著高于同地区正常人群的(7.7±2.3)ng/L,P<0.05和P<0.01。盘山县正常人群的Tg水平显著高于彰武县正常人群(P<0.01)。在结节性甲状腺肿人群中,Tg水平与甲状腺体积呈正相关。
轻度碘缺乏地区甲状腺肿的发生与TSH刺激无直接关系。非毒性甲状腺肿呈现甲状腺自主功能。结节性甲状腺肿血清Tg升高的主要原因可能是大胶质滤泡变性和滤泡结构破坏导致Tg漏入血液循环,而非甲状腺体积增大及其自主功能。
