Both a missed diagnosis of epilepsy, a wrong diagnosis of epilepsy as something else, and a misdiagnosis of something else as epilepsy can have serious consequences for the patient, e.g. therapeutic deviations, or physical, physical and social complications. Not all possible but some important problems of differential diagnosis are discussed, including both typical misdiagnoses of epileptic seizures as cardiac synkope or pseudoepileptic fits, oversight of subtle manifestions of epilepsy such as absences, juvenile myoclonic epilepsy, and isolated auras, and misdiagnoses as epilepsy of migraine variants, syncope, episodic ataxias, narcolepsy, and parasomnias.
