The necessity of recording data within an organization-oriented information system demands the usage of forms. Based on the experiences made at the Klinikum Steglitz some basic principles for planning and design of forms within a hospital information system are derived. Referring to the concept of information flow 5 main types of information processes and coordinated forms are differentiated. Serious problems for the design of forms within a hospital information system result from psychosocial factors.
