Abstract

AIM

To investigate the value of prospective electrocardiogram (ECG)-gated dual-source computed tomography (DSCT) in the diagnosis of coarctation of the aorta (CoA).

MATERIALS AND METHODS

Seventeen patients clinically suspected of having CoA underwent prospective ECG-gated DSCT angiography and transthoracic echocardiography (TTE). Surgery was performed in all patients. The diagnostic accuracy of DSCT angiography and TTE was compared with the surgical findings as the reference standard. Image quality was evaluated using a five-point scale. Effective radiation dose was calculated from the dose-length product (DLP).

RESULTS

CoA was diagnosed in 17 patients by DSCT angiography and in 16 patients by TTE. A total of 46 separate cardiovascular abnormalities were confirmed by surgical findings. The diagnostic accuracy of DSCT angiography and TTE was 96.32% and 97.06%, respectively. There was no significant difference in the diagnostic accuracy between DSCT angiography and TTE (χ(2) = 0, p > 0.05). The mean score of image quality was 4.2 ± 0.8. The mean effective dose was 0.69 ± 0.09 mSv.

CONCLUSION

Prospective ECG-gated DSCT with a low radiation dose is a valuable technique in the diagnosis of CoA in infants and children.

目的

探讨前瞻性心电图门控双源 CT(DSCT)在诊断主动脉缩窄(CoA)中的应用价值。

材料与方法

对 17 例临床疑诊 CoA 的患者进行前瞻性心电图门控 DSCT 血管造影和经胸超声心动图(TTE)检查。所有患者均接受手术治疗。以手术结果为参考标准,比较 DSCT 血管造影和 TTE 的诊断准确性。采用五分制评估图像质量。根据剂量长度乘积(DLP)计算有效辐射剂量。

结果

DSCT 血管造影诊断出 17 例 CoA,TTE 诊断出 16 例。手术结果共证实 46 种不同的心血管异常。DSCT 血管造影和 TTE 的诊断准确率分别为 96.32%和 97.06%。DSCT 血管造影和 TTE 的诊断准确率无显著差异(χ(2) = 0,p > 0.05)。图像质量平均得分为 4.2 ± 0.8。平均有效剂量为 0.69 ± 0.09 mSv。

结论
