Esophageal cancer (EC) patients presenting a local recurrence following trimodality therapy (chemoradiaton and surgery) have limited palliative treatment options when the three major modalities of therapy have been exhausted. In addition, some patients experience a local recurrence or develop a metachronous cancer in a previously irradiated site, without evidence of systemic disease. For these patients there is a potential for cure, although the risk of further distant recurrences remains high.
We report of a successful concomitant chemo/SBRT treatment in a case of locally advanced metachronous squamous cervical EC, which was diagnosed in a patient previously treated with trimodality therapy for a squamous tonsillar carcinoma.
Chemo/SBRT seems to be a reasonable salvage option for patients without distant metastases who have exhausted standard therapies.
Our experience also suggests that a concomitant chemo/SBRT treatment appears to be either feasible or effective and chemo/SBRT can be considered also in selected patients affected by EC with squamous histology and with neoplastic infiltration of the trachea.
接受三联疗法(化疗、放疗和手术)后出现局部复发的食管癌(EC)患者,当这三种主要治疗方式都已用尽时,姑息治疗选择有限。此外,一些患者在先前接受过放疗的部位出现局部复发或发生异时性癌,且无全身疾病证据。对于这些患者,尽管远处进一步复发的风险仍然很高,但仍有治愈的可能。
我们报告了一例局部晚期异时性宫颈鳞状EC患者成功接受同步化疗/立体定向体部放疗(SBRT)治疗的病例,该患者先前因扁桃体鳞状癌接受过三联疗法治疗。
对于那些已用尽标准疗法且无远处转移的患者,化疗/SBRT似乎是一种合理的挽救治疗选择。
我们的经验还表明,同步化疗/SBRT治疗似乎可行或有效,对于某些患有鳞状组织学EC且有气管肿瘤浸润的患者,也可考虑采用化疗/SBRT。
