Abstract

AIMS OF STUDY

The aim of the study was to establish the long term efficacy of a perioperative antibiotic protocol combined with antibacterial mouthwashes in preventing osteoradionecrosis (ORN).

MATERIALS AND METHODS

Irradiated head and neck cancer patients reporting for dental extractions were prospectively enrolled to the study between January 2002 and December 2009. Selection criteria for the patients included the presence of nonrestorable tooth/teeth in the field of radiation, latency period of 6 months since completion of radiotherapy, radiation dosages >60 Gy, and availability for follow-up. Starting from 10 days preextraction, the patients were prescribed 8th hourly oral amoxicillin 500 mg along with 12th hourly mouthwashes using 10 ml of undiluted chlorhexidine gluconate 0.2% solution. The same prescription was continued for 7 days postextraction. All patients were followed-up at regular intervals until December 2013.

RESULTS

A total of 89 patients (55 male and 34 female) underwent extractions of teeth which were present in the radiation field. Mean age of the patients was 41.8 years (range 36-54 years) and extractions were done between 12 and 33 months (mean - 15 months) postradiation therapy. Altogether, 232 teeth were extracted (maxilla - 78/mandible - 154) at an average of 2.6 teeth per patients. After a mean follow-up period of 63 months (range 48-123 months) there were no reported cases of ORN.

CONCLUSION

Based on the results of this study, perioperative oral antibiotics in combination with antibacterial mouthwashes are effective in preventing ORN following dental extractions in irradiated patients.

本研究的目的是确定围手术期抗生素方案联合抗菌漱口水在预防放射性骨坏死(ORN)方面的长期疗效。

材料与方法

2002年1月至2009年12月期间,前瞻性招募了因牙齿拔除前来就诊的头颈癌放疗患者。患者的入选标准包括放疗区域存在无法修复的牙齿、放疗结束后6个月的潜伏期、放疗剂量>60 Gy以及能够进行随访。从拔牙前10天开始,患者每8小时口服500毫克阿莫西林,并每12小时使用10毫升未稀释的0.2%葡萄糖酸氯己定溶液进行漱口水护理。拔牙后同样的处方持续7天。所有患者定期随访至2013年12月。

结果

共有89例患者(55例男性和34例女性)拔除了放疗区域内的牙齿。患者的平均年龄为41.8岁(范围36 - 54岁),拔牙在放疗后12至33个月(平均 - 15个月)进行。总共拔除了232颗牙齿(上颌骨 - 78颗/下颌骨 - 154颗),平均每位患者2.6颗牙齿。平均随访63个月(范围48 - 123个月)后,未报告ORN病例。

结论

基于本研究结果,围手术期口服抗生素联合抗菌漱口水在预防放疗患者拔牙后ORN方面是有效的。
