Collecting data and information is a key element in ensuring patient safety. The most reliable healthcare organizations are those that put the measurement and monitoring of safety at the center of their attention. The Italian legislation offers a reference model for measuring safety applicable at all relevant levels (national, regional and health organizations) based on the acquisition of information from a variety of sources. The information deriving from the sources taken into consideration must be used both for the quantification of events and for the definition and implementation of improvement actions.
