Abstract

PURPOSE

To observe the performance of cyclosporine A (CsA)-loaded intraocular lens (IOLs) implanted into rabbit eyes.

METHODS

To prepare a PLGA-based CsA-sustained release IOLs and study the in vitro drug release. Forty-two New Zealand white rabbits were randomly and equally divided into three groups, and all right eyes underwent phacoemulsification. In group A, a common polymethylmethacrylate (PMMA) IOLs was implanted, while polylactide-glycoli acid (PLGA-loaded)-PMMA-IOLs was implanted in group B, and CsA-PLGA-PMMA-IOLs was implanted in group C. All experimental eyes were examined by slit-lamp microscopy. In addition, fundoscopy and the number of corneal endothelial cells, anterior chamber flare grading, and the number of aqueous humor cells were assessed at different time points post-surgery. The wet lens capsule was weighed and histological examination was performed 6 months post-operation.

RESULTS

In the early post-operative period, the inflammatory reaction of anterior chamber in groups A and B were more severe than group C. The initial appearance of PCO in group C was much later than the other two groups ( = 68.91;  = 0.000), and PCO grade in group C was much lower than the other two groups (χ = 36.07;  = 0.000). The mean weights of wet lens capsules in groups A and B were significantly heavier than group C ( = 134.88;  = 0.00). Histological observation showed no obvious toxic reaction in the intraocular tissues of the CsA-PLGA-PMMA-IOLs group, and the proliferation and accumulation of lens epithelial cells in groups A and B were greater than in group C.

CONCLUSION

CsA-sustained release IOLs can effectively prevent PCO in rabbit eyes without defined intraocular toxicity.

目的

观察载环孢素 A(CsA)的眼内透镜(IOL)在兔眼内的性能。

方法

制备基于聚乳酸-羟基乙酸(PLGA)的 CsA 持续释放 IOL,并研究其体外药物释放。42 只新西兰白兔随机均分为三组,右眼均行超声乳化白内障吸除术。A 组植入普通聚甲基丙烯酸甲酯(PMMA)IOL,B 组植入载 PLA-PMMA-IOL,C 组植入 CsA-PLGA-PMMA-IOL。所有实验组兔眼均行裂隙灯显微镜检查,术后不同时间点行眼底检查、角膜内皮细胞计数、前房闪辉分级和房水细胞计数,并于术后 6 个月称重湿镜片囊,行组织学检查。

结果

术后早期,A 组和 B 组前房炎症反应均较 C 组重。C 组后发性白内障(PCO)的初始出现时间明显晚于其他两组(Z=-68.91,P=0.000),且 C 组 PCO 分级明显低于其他两组(X2=36.07,P=0.000)。A 组和 B 组湿镜片囊平均重量明显重于 C 组(Z=134.88,P=0.00)。组织学观察发现,CsA-PLGA-PMMA-IOL 组眼内组织未见明显毒性反应,A 组和 B 组晶状体上皮细胞增生和堆积较 C 组明显。

结论

载 CsA 的持续释放 IOL 能有效预防兔眼 PCO,且无明确眼内毒性。
