Although it has been observed that the triglycerides and glucose (TyG) index, a biomarker of insulin resistance, is associated with severity and morbidity by COVID-19, evidence is still scarce. Therefore, the objective of this study was to determine whether the TyG index is associated with both the degree of severity and mortality by acute respiratory distress syndrome (ARDS) in patients with COVID-19. Men and women aged 20 years or more with diagnosis of COVID-19 were included in a case-control study. Exclusion criteria were pregnancy, cancer, autoimmune diseases, autoimmune treatment, and incomplete data. Patients with severe COVID-19 ARDS were allocated into the case group, and those with mild or moderate COVID-19 ARDS in the control group. COVID-19 was defined by a positive reverse transcriptase-polymerase chain reaction test for SARS-CoV-2, and ARDS was defined according to the Berlin criteria. A total of 206 patients were included and allocated into the case ( = 103) and control ( = 103) groups. The logistic regression analysis adjusted by age, sex, and body mass index showed that the TyG index is significantly associated with moderate [odds ratio (OR) = 6.0; 95% confidence interval (CI): 1.1-30.6] and severe (OR = 9.5; 95% CI: 2.4-37.5) COVID-19 ARDS, and death (OR = 10.1; 95% CI: 2.2-46.5). The results of our study show a significant and independent association of the TyG index with ARDS and mortality in patients with COVID-19.
尽管有观察表明,作为胰岛素抵抗生物标志物的甘油三酯与葡萄糖(TyG)指数与新冠病毒疾病(COVID-19)的严重程度及发病率相关,但相关证据仍然匮乏。因此,本研究的目的是确定TyG指数是否与COVID-19患者急性呼吸窘迫综合征(ARDS)的严重程度及死亡率相关。年龄在20岁及以上且确诊为COVID-19的男性和女性被纳入一项病例对照研究。排除标准包括妊娠、癌症、自身免疫性疾病、免疫治疗以及数据不完整。患有重症COVID-19 ARDS的患者被分配到病例组,而患有轻症或中症COVID-19 ARDS的患者被分配到对照组。COVID-19通过针对严重急性呼吸综合征冠状病毒2(SARS-CoV-2)的逆转录聚合酶链反应检测呈阳性来定义,ARDS则根据柏林标准进行定义。总共206名患者被纳入并分配到病例组(n = 103)和对照组(n = 103)。经年龄、性别和体重指数校正的逻辑回归分析表明,TyG指数与中度(比值比[OR]=6.0;95%置信区间[CI]:1.1 - 30.6)和重度(OR = 9.5;95% CI:2.4 - 37.5)COVID-19 ARDS以及死亡(OR = 10.1;95% CI:2.2 - 46.5)显著相关。我们的研究结果表明,TyG指数与COVID-19患者的ARDS及死亡率之间存在显著且独立的关联。
