DRG(Diagnosis Related Groups,诊断相关组)和DIP(Diagnosis Intervention Packet,区域点数法总额预算和按病种分值付费)是目前国内推行的主要医保支付改革模式,旨在通过标准化、精细化管理来控制医疗成本、提高效率和医疗质量。

OBJECTIVES: This meta-analysis aimed to evaluate the value of the chest digital drainage system for the postoperative management of patients who have undergone pulmonary resection. METHODS: We searched the PubMed, EMBASE, the Cochrane Library, and Web of Science databases for included randomized controlled trials (RCTs) on the application of digital drainage systems versus the analog drainage system for patients with lung disease after pulmonary resection. Dichotomous variables were evaluated using risk ratios (RRs) and 95% confidence intervals (CIs), and mean and standardized mean differences (MDs and SMDs, respectively) with 95% CIs were used to calculate continuous variables. Statistical analyses were performed using Stata and RevMan software. RESULTS: In total, 12 RCTs involving 2000 patients were analyzed. Significant differences in duration of chest tube placement (SMD = -0.49; 95% CI = -0.78 to -0.20), length of hospital stay (MD =-0.79 days; 95% CI = -1.24 to -0.34), and number of chest tube clamping tests (RR = 0.74; 95% CI = 0.36-1.49) were observed between the two groups, which did not significant differ in the occurrence of prolonged air leak or cardiopulmonary complication rate. CONCLUSIONS: The digital chest drainage system is mainly advantageous in the duration of chest tube placement, length of hospital stay, and number of chest tube clamping tests. Future research should evaluate the requirements and economic impact of using digital system in routine clinical practice.

In this systematic review with meta-analysis, we sought to determine the current strength of evidence for or against digital and traditional chest drainage systems following pulmonary surgery with regards to hard clinical end points and cost-effectiveness. PubMed, EMBASE and Web of Science were searched from their inception to 31 July 2017. The weighted mean difference (WMD) and the risk ratio were used for continuous and dichotomous outcomes, respectively, each with 95% confidence intervals (CIs). The heterogeneity and risk of bias were also assessed. A total of 10 randomized controlled trials enrolling 1268 patients were included in this study. Overall, digital chest drainage reduced the duration of chest tube placement (WMD -0.72 days; 95% CI -1.03 to -0.40; P < 0.001), length of hospital stay (WMD -0.97 days; 95% CI -1.46 to -0.48; P < 0.001), air leak duration (WMD -0.95 days; 95% CI -1.51 to 0.39; P < 0.001), and postoperative cost (WMD -443.16 euros; 95% CI -747.60 to -138.73; P = 0.004). However, the effect differences between the 2 groups were not significant for the duration of a prolonged air leak and the percentage of patients discharged home on a device. The stability of these studies was strong. No publication bias was detected. It may be necessary to use a digital chest drainage system for patients who underwent pulmonary surgery to reduce the duration of chest tube placement, length of hospital stay and air leak duration.

The proper management of chest tubes for patients undergoing lung resection is very important to prevent postoperative complications. Although it is desired to have evidence-based consensus for the management of chest tubes, currently the protocols vary among institutions. This article reviewed some recent literatures and our opinions regarding postoperative management of chest tubes. The use of 1 chest tube has been recently preferred compared to 2 chest tubes. However, the use of 2 chest tubes are required when postoperative air leakage, massive fluid discharge, or hemorrhage is concerned after peeling the firm and broad range of pleural adhesions. The chest tube is inserted through the skin incision through an access port and upper edge of the rib at an intercostal space from the skin incision, and then, commonly attached to the chest drainage unit employing a 3-bottle system. Although continuous suction with water sealing is usually utilized, a randomized controlled trial revealed that continuous suction was not better than water sealing without suction in terms of duration of air leakage, chest drainage, and hospital stay. Chest tubes can be safely removed with both no air leakage during coughing as well as daily drainage volume of up to 200 ml. According to a systematic review and meta-analysis, recently developed digital chest drainage system, with which the pleural pressure can be constantly maintained and air leakage can be evaluated objectively, proved to reduce the risk of prolonged air leakage and to shorten the duration of chest drainage and hospital stay after lung resection when compared with the traditional analog drainage system. Prolonged postoperative air leakage is usually treated with chemical pleurodesis using autologous blood, minocycline, or OK -432. Some studies demonstrated that pleurodesis with 50% glucose solution was a safe and effective treatment option.

J Cardiothorac Surg. 2019 Jan 18;14(1):13. doi: 10.1186/s13019-019-0842-x.
BACKGROUND: Several randomized controlled trials (RCTs) and observational studies have compared the efficacy of digital chest drainage system versus traditional chest drainage system. However, the results were inconsistent. METHODS: We searched the Web of Science and Pubmed for observational studies and RCTs that compared the effect of digital chest drainage system with traditional chest drainage system after pulmonary resection. Eight studies (5 randomized control trails and 3 observational studies) comprising 1487 patients met the eligibility criteria. RESULTS: Compared with the traditional chest drainage system, digital chest drainage system reduced the risk of prolonged air leak (PAL) (RR = 0.54, 95%CI 0.40-0.73, p < 0.0001), and shortened the duration of chest drainage (SMD = - 0.35, 95%CI -0.60 - -0.09, p = 0.008) and length of hospital stay (SMD = - 0.35, 95%CI -0.61 - -0.09, p = 0.007) in patients after pulmonary resection. CONCLUSIONS: Digital chest drainage system is expected to benefit patients to attain faster recovery and higher life quality as well as to reduce the risk of postoperative complications. Further RCTs with larger sample size are still needed to more clearly elucidate the advantages of digital chest drainage system.
