The challenge of working together: documentation collaboration.

Duplicating the preoperative assessment kept the staff working parallel, instead of intersecting or collaborating. The process of promoting better communication via collaborative documentation required staff to reflect on their roles--not as members of the PACU or OR team, but as part of the group giving surgical care.
