Screening with Fecal Occult Blood Test (FOBT) fails to adequately select patients who could have colorectal tumors and should be sent to colonoscopy. The latter investigation should be considered as the first intention examination in every person aged more than 50 years old (or, 40 years if there is a family history of elevated cancer risk). Colonoscopy will allow to identify the numerous patients who, in spite of significant disease, would have a negative FOBT.
