The occurrence of an abnormal cardiovascular response during exercise in patients with chronic heart failure is well known. Post-exercise blood pressure response is also useful in assessing the severity of heart failure and impaired exercise capacity. This study evaluated the prognostic significance of post-exercise blood pressure response in patients with dilated cardiomyopathy.
Thirty patients with dilated cardiomyopathy (left ventricular ejection fraction: 32 +/- 9%) were studied and the relationship between post-exercise blood pressure response and cardiac events (sudden death, heart failure death and readmission for heart failure) were evaluated. The post-exercise blood pressure response was defined as PBP3 (systolic blood pressure at 3 min after exercise divided by peak systolic blood pressure during exercise).
Seven cardiac events (one sudden death, two deaths for heart failure and four readmissions from heart failure) were observed during the follow-up period (3.3 +/- 1.8 years). The PBP3 in patients with these cardiac events was higher than that in patients without cardiac events (0.95 +/- 0.09 vs 0.84 +/- 0.10, p < 0.05). The area under the curve for the receiver-operating characteristic curve with PBP3 used to predict the cardiovascular events was 0.79 (95% confidence interval: 0.62-0.97, p = 0.02).
Post-exercise blood pressure response is a simple and useful predictor of adverse cardiac events in patients with dilated cardiomyopathy.
慢性心力衰竭患者运动期间出现异常心血管反应是众所周知的。运动后血压反应对于评估心力衰竭的严重程度和运动能力受损情况也很有用。本研究评估了扩张型心肌病患者运动后血压反应的预后意义。
对30例扩张型心肌病患者(左心室射血分数:32±9%)进行研究,评估运动后血压反应与心脏事件(猝死、心力衰竭死亡和因心力衰竭再次入院)之间的关系。运动后血压反应定义为PBP3(运动后3分钟收缩压除以运动期间收缩压峰值)。
在随访期(3.3±1.8年)内观察到7例心脏事件(1例猝死、2例心力衰竭死亡和4例因心力衰竭再次入院)。发生这些心脏事件的患者的PBP3高于未发生心脏事件的患者(0.95±0.09对0.84±0.10,p<0.05)。用于预测心血管事件的PBP3的受试者工作特征曲线下面积为0.79(置信区间95%:0.62 - 0.97,p = 0.02)。
运动后血压反应是扩张型心肌病患者不良心脏事件的一个简单且有用的预测指标。
