As a medical advisor specialized in palliative care, I think the following points should be considered regarding management of patients after discharge from a tertiary hospital: 1. Information that we should get before the interview and examination, 2. Assessment of symptoms, 3. Medications for the management of pain, 4. Supportive care to cope with psychosocial problems derived from the management of physical symptoms, 5. Help to continue with activities of daily life and to cope with the cancer at home, and 6. Characterize of the medical advisor as a member of an interdisciplinary team.
作为一名专门从事姑息治疗的医学顾问,我认为在三级医院出院后对患者的管理应考虑以下几点:1. 访谈和检查前我们应获取的信息;2. 症状评估;3. 疼痛管理的药物;4. 应对因身体症状管理而产生的心理社会问题的支持性护理;5. 帮助患者继续日常生活活动并在家中应对癌症;6. 将医学顾问定位为跨学科团队的一员。
