We are reporting the clinical case of a 56 years-old female who was diagnosed and operated of a stage V Clark melanoma in temporal zone that showed, 21 months after the surgery, several left cervical lymph nodes. The F.N.A.B. informed as melanoma metastases, result that was confirmed by AP study then a functional neck dissection. Later she has followed treatment by Oncology-Radiotherapy. Melanoma can isolate between a 5%-10% from all the cases of cervical lymph node metastases but in most of the patients it is not possible to find the primary lesion. Its vital prognosis is bad and the role of adjuvant irradiation remains controversial.
