To investigate the expression of the matrix metalloproteinases (MMP)-2, -3, and 9, tissue inhibitor of matrix metalloproteinase-1 (TIMP-1), fibronectin (FN), and tenascin-C (TN-C) in the myocardium, and their relationship with the myocardial remodeling and heart function in the patients with congestive heart failure (CHF).
A few tissues of papillary muscles of left ventricle were taken from 39 patients with mitral insufficiency during mitral valve replacement, and from 8 organ donors who had died of accidents to undergo pathological examination and to detect the protein expression of MMP-2, -3, and 9, and TIMP-1 by immunoprecipitation and the expression of FN and TN-C by immunofluorescence test. Peripheral blood samples were collected from these 39 patients and 30 normal controls to examine the plasma MMPs and TIMP-1 by ELISA.
Typical myocardial remodeling was seen in the myocardial tissues of the CHF patients. The plasma levels of MMP-2, -3, and -9 of the CHF patients in the cardiac function classes II - IV were all significantly higher than those of the normal controls (P < 0.05 or P < 0.01) with a tendency to increase with the deterioration of heart function, The plasma level of TIMP-1 of the CHF patients in the cardiac function classes II - IV were all significantly higher than that of the normal controls (P < 0.05 or P < 0.01) with a tendency to decrease with the deterioration of heart function. The protein expression levels of MMP-2, -3, and -9 in the myocardium of the CHF patients in the cardiac function classes III - IV were all significantly higher than those of the normal controls (P < 0.05 or P < 0.01) with a tendency to increase with the deterioration of heart function. The protein expression levels of TIMP-1 in the myocardium of the CHF patients in the cardiac function classes III - IV were all significantly lower than that of the normal controls (P < 0.05 or P < 0.01) with a tendency to decrease with the deterioration of heart function. The protein expression levels of FN in the myocardium of the CHF patients in the cardiac function classes II - IV were all significantly lower than that of the normal controls (all P < 0.05) with a tendency to decrease with the deterioration of heart function. The protein expression levels of TN-C in the myocardium of the CHF patients in the cardiac function classes II - IV were all significantly higher than that of the normal controls (all P < 0.01) with a tendency to increase with the deterioration of heart function.
The increasing expression of MMP-2, -3, and -9 and the decreasing expression of TIMP-1 contribute to the myocardial remodeling, thus changing the extracellular matrix in the myocardium tissue and leading to the development and progression of CHF.
探讨基质金属蛋白酶(MMP)-2、-3、-9、基质金属蛋白酶组织抑制因子-1(TIMP-1)、纤连蛋白(FN)和腱糖蛋白-C(TN-C)在充血性心力衰竭(CHF)患者心肌中的表达及其与心肌重构和心功能的关系。
选取39例二尖瓣置换术中二尖瓣关闭不全患者的部分左心室乳头肌组织,以及8例因意外死亡的器官捐献者的组织进行病理检查,采用免疫沉淀法检测MMP-2、-3、-9和TIMP-1的蛋白表达,采用免疫荧光法检测FN和TN-C的表达。采集这39例患者和30例正常对照者的外周血样本,采用酶联免疫吸附测定(ELISA)法检测血浆MMPs和TIMP-1水平。
CHF患者心肌组织出现典型的心肌重构。心功能Ⅱ-Ⅳ级CHF患者血浆MMP-2、-3和-9水平均显著高于正常对照者(P<0.05或P<0.01),且随心功能恶化呈升高趋势;心功能Ⅱ-Ⅳ级CHF患者血浆TIMP-1水平均显著高于正常对照者(P<0.05或P<0.01),且随心功能恶化呈降低趋势。心功能Ⅲ-Ⅳ级CHF患者心肌中MMP-2、-3和-9的蛋白表达水平均显著高于正常对照者(P<0.05或P<0.01),且随心功能恶化呈升高趋势;心功能Ⅲ-Ⅳ级CHF患者心肌中TIMP-1的蛋白表达水平均显著低于正常对照者(P<0.05或P<0.01),且随心功能恶化呈降低趋势。心功能Ⅱ-Ⅳ级CHF患者心肌中FN的蛋白表达水平均显著低于正常对照者(均P<0.05),且随心功能恶化呈降低趋势。心功能Ⅱ-Ⅳ级CHF患者心肌中TN-C的蛋白表达水平均显著高于正常对照者(均P<0.01),且随心功能恶化呈升高趋势。
MMP-2、-3、-9表达增加及TIMP-1表达降低促进心肌重构,从而改变心肌组织细胞外基质,导致CHF的发生发展。
