We report on a rare case of a 61-year-old female patient with tetralogy of Fallot who had a late radical repair. The patient had no previous surgical interventions, and her postoperative course was uneventful. One year after the repair, the patient is doing well and has no symptoms of cardiac failure. Surgical treatment greatly benefited our patient by improving her functional status and extending her life expectancy.
