To present a case report of severe post-partum bleeding after caesarean section and successful treatment using recombinant factor VIIa.

A 29-year old secundigravida presented with major post-partum bleeding after caesarean section due to uterine atony. The patient developed hemorrhagic shock, associated with disseminated intravascular coagulation (DIC). Treatment with uterotonic drugs, prostaglandins and hysterectomy failed to control diffuse bleeding. Recombinant factor VIIa (90 microg/kg intravenous injection) was given as a final attempt to control the bleeding. The response to treatment was rapid, with control of the bleeding and resolution of the coagulopathy.
This case suggests a potential role of recombination factor VIIa in the treatment of severe post-partum bleeding associated with DIC.
报告一例剖宫产术后严重产后出血并使用重组凝血因子VIIa成功治疗的病例。

一名29岁经产妇因子宫收缩乏力在剖宫产术后出现严重产后出血。患者发生失血性休克，并伴有弥散性血管内凝血（DIC）。使用宫缩剂、前列腺素及子宫切除术均未能控制弥漫性出血。作为控制出血的最后尝试，给予重组凝血因子VIIa（90微克/千克静脉注射）。治疗反应迅速，出血得到控制，凝血病得以缓解。
该病例提示重组凝血因子VIIa在治疗与DIC相关的严重产后出血中可能具有一定作用。
