Abstract

Primary total hip arthroplasty (THA) has become a routine procedure with a high level of reproductibility, enabling improved function in the great majority of patients. Main modern concepts in THA have been established for more than forty years ago. An effort has been made to improve prosthetic stability and to reduce aseptic loosening, due to particle wear. Thus, several ways have been explored, including minimizing the surgical approach, improving wear resistance of the bearing surface, new methods of fixation, and innovative mechanical concepts. However, the clinical benefits of some of these techniques are still to be established. These technical advances are detailed in this article through a review of the recent literature, indicating the pros and cons for each concept.

初次全髋关节置换术(THA)已成为一种具有高度可重复性的常规手术,能使绝大多数患者的功能得到改善。THA的主要现代理念在四十多年前就已确立。人们一直在努力提高假体稳定性并减少因颗粒磨损导致的无菌性松动。因此,已经探索了多种方法,包括尽量减少手术入路、提高关节面的耐磨性、新的固定方法以及创新的机械理念。然而,其中一些技术的临床益处仍有待确定。本文通过回顾近期文献详细介绍了这些技术进展,指出了每个理念的优缺点。
