After a short review on literature the late result of an operated elbow with a follow-up time of 23 years is reported. The actual problems of this rare condition are discussed. Congenital hyperlaxity, a shallow semilunar notch and traumatic sequelae after previous dislocations reduce joint stability. Arthroscopy, X-ray tomography and MRI update accurate preoperative diagnosis. Early operation is recommended to reduce further damage in the joint. Nowadays well proved operative procedures provide good postoperative results.
