The Long Term Conditions Delivery Support (LTC DS) Team.

The LTC DS Team is a small, NHS-funded organization with one team member based in each of the ten Strategic Health Authority regions around the country. Over the past few years, the direction and focus of our work has had a few twists and turns, not least of all owing to the recent change in government. Our remit is, broadly speaking, to support the implementation of government policy, and we do this by supporting service improvement and innovation for people with long-term conditions, with an emphasis on neurology.
