To identify the proportion of low adiponectin level and to evaluate the role of low adiponectin level, age, body mass index, and waist circumference on the occurrence of metabolic syndrome in women with insulin resistance.
The study was done by a cross-sectional survey on adult women aged 30-60 years. The study was conducted at Fatmawati Hospital-Jakarta from January to March 2008 with a total sample of 33. The sample was selected consecutively based on the presence of metabolic syndrome according to the IDF 2005 criteria. The examination of plasma adiponectin level was done by ELISA method. Insulin resistance was defined if HOMA IR > or = 2 (calculated by fasting insulin (microU/mL) x Fasting blood sugar (mmol/L) divided by 22.5). Association of low adiponectin level with insulin resistance was evaluated by calculating the Odds Ratio.
Seventeen of 33 subjects with metabolic syndrome have insulin resistance, in which 7 of them (41.18%) show low adiponectin level. While in 16 subjects without insulin resistance, only one subject (6.25%) has low adiponectin level. From 8 subjects with low adiponectin level, 7 of them (87.5%) have insulin resistance. In other words, low adiponectin level is associated with increased risk of insulin resistance (Odds Ratio 10.5, P = 0.040 (CI 95% : 1.12-98.91).
Low adiponectin concentration increases the risk of developing insulin resistance much more than normal adiponectin level.
确定脂联素水平低下的比例,并评估低水平脂联素、年龄、体重指数和腰围在胰岛素抵抗女性发生代谢综合征中的作用。
本研究采用横断面调查方法,针对30至60岁的成年女性。研究于2008年1月至3月在雅加达法塔瓦蒂医院进行,共纳入33例样本。根据2005年国际糖尿病联盟(IDF)标准,基于代谢综合征的存在情况连续选取样本。采用酶联免疫吸附测定(ELISA)法检测血浆脂联素水平。若稳态模型评估的胰岛素抵抗指数(HOMA-IR)≥2(通过空腹胰岛素(微单位/毫升)×空腹血糖(毫摩尔/升)除以22.5计算得出),则定义为胰岛素抵抗。通过计算比值比评估低水平脂联素与胰岛素抵抗的关联。
33例患有代谢综合征的受试者中,17例存在胰岛素抵抗,其中7例(41.18%)脂联素水平低下。而在16例无胰岛素抵抗的受试者中,仅有1例(6.25%)脂联素水平低下。在8例脂联素水平低下的受试者中,7例(87.5%)存在胰岛素抵抗。换言之,低水平脂联素与胰岛素抵抗风险增加相关(比值比为10.5,P = 0.040(95%置信区间:1.12 - 98.91))。
低水平脂联素浓度相比正常脂联素水平更易增加发生胰岛素抵抗的风险。
