No consensus has been reached on the optimal treatment for patients with nasopharyngeal cancer (NPC) who develop distant metastasis after initial radiation therapy (RT).
Two hundred eighty-two patients with NPC received curative RT (+/- chemotherapy). Forty-six patients (16.3%) who developed distant metastasis as first failure formed the study group for the current analysis.
The median interval from initial RT until distant metastasis was 11.6 months. With a median follow-up of 30 months among survivors, overall survival (OS) rates at 2 and 5 years were 53.7% and 30.5%, respectively. On multivariate analyses, 18F-fluorodeoxyglucose (FDG)-positron emission tomography (PET)/CT for initial staging, RT plus chemotherapy as initial treatment, metastatic lesion number <6, disease-free interval >9 months, distant metastasis only to lungs, and treatment with curative intent after distant metastasis were predictive of significantly better OS.
Combined with FDG-PET/CT, an aggressive treatment approach using locoregional modalities might be beneficial to patients with NPC with favorable prognostic factors, even after distant metastasis. © 2015 Wiley Periodicals, Inc. Head Neck 38: E293-E299, 2016.
对于鼻咽癌(NPC)患者在初始放射治疗(RT)后发生远处转移的最佳治疗方法,尚未达成共识。
282例鼻咽癌患者接受了根治性放疗(±化疗)。46例(16.3%)作为首次失败发生远处转移的患者构成了当前分析的研究组。
从初始放疗到远处转移的中位间隔时间为11.6个月。幸存者的中位随访时间为30个月,2年和5年的总生存率(OS)分别为53.7%和30.5%。多因素分析显示,初始分期采用18F-氟脱氧葡萄糖(FDG)-正电子发射断层扫描(PET)/CT、初始治疗采用放疗加化疗、转移病灶数<6、无病间期>9个月、仅肺转移以及远处转移后采用根治性治疗,对总生存率有显著更好的预测作用。
结合FDG-PET/CT,即使在远处转移后,对具有良好预后因素的鼻咽癌患者采用局部区域治疗手段的积极治疗方法可能有益。©2015威利期刊公司。《头颈》38:E293-E299,2016。
