The effects of captopril on renal function are evaluated in hypertensive patients with impaired renal function previously entered in a worldwide surveillance study. For the overwhelming majority of patients, control of BP was accompanied by stable or improved renal function. To date, the major adverse renal reactions have been a somewhat increased risk of worsening proteinuria among these patients and the well-documented acute decrease in renal function that may occur in patients with bilateral renal artery stenosis.
