To investigate clinical outcomes of total knee arthroplasty with ultra-congruent insert(UC) or posterior stability insert (PS). A retrospective study was performed on 97 patients (17 males, 80 females, 119 knees) with knee osteoarthritis who received total knee arthroplasty. Of the patients, 42 cases (50 knees) received UC protheses and 55 cases (69 knees) received PS protheses in total knee arthroplasty by the same surgeon in Peking University Third Hospital from March 2015 to November 2015. The data including age, gender, body mass index (BMI), range of motion (ROM), Western Ontario and McMaster Universities (WOMAC) osteoarthritis index and Hospital for Special Surgery Knee Score (HSS) before and after the surgery were collected and compared in the two groups. The data were compared between the two groups by single-sample test. There was no significant differences in age, gender, BMI, ROM, HHS and WOMAC scores between the two groups before operation(all 0.05). At 3 months and 2 years of follow-up, ROM, HSS and WOMAC scores were significantly improved in the two groups(all 0.05), but there was no statistical difference between the two groups(-0.303, -1.593, -0.500, all 0.05). One patient with PS prosthesis recieved revision surgery due to prothesis loosening; and no complication found in the UC group(χ(2)=0.731, 0.05). There is no obvious difference between PS and UC in TKA, UC insert seems to be a practical alternative to the PS.
为研究采用超匹配型假体(UC)或后稳定型假体(PS)进行全膝关节置换术的临床疗效。对97例(男性17例,女性80例,共119膝)膝关节骨关节炎患者进行了全膝关节置换术的回顾性研究。2015年3月至2015年11月,在北京大学第三医院,由同一位外科医生对这些患者进行全膝关节置换术,其中42例(50膝)采用UC假体,55例(69膝)采用PS假体。收集并比较两组患者手术前后的年龄、性别、体重指数(BMI)、活动范围(ROM)、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)以及特种外科医院膝关节评分(HSS)。两组数据采用单样本检验进行比较。两组患者术前在年龄、性别、BMI、ROM、HHS和WOMAC评分方面均无显著差异(均P>0.05)。在随访3个月和2年时,两组患者的ROM、HSS和WOMAC评分均显著改善(均P<0.05),但两组之间无统计学差异(t=-0.303、-1.593、-0.500,均P>0.05)。1例采用PS假体的患者因假体松动接受了翻修手术;UC组未发现并发症(χ²=0.731,P>0.05)。全膝关节置换术中PS和UC之间无明显差异,UC假体似乎是PS的一种实用替代方案。
