To evaluate the efficacy of bundle intervention on healthcare-associated (HA) methicillin-resistant (MRSA) infection in the neonatal intensive care unit (NICU). In this study, 11,277 infants having undergone treatment at the NICU in Xiamen, China, from January 2014 to February 2017 were recruited. We retrospectively reviewed patients' demographic and clinical information. Patients from 2014 to 2015 were treated as the control group and those from 2016 to 2017 were classified as the experimental group. Bundle intervention measures were performed, including screening for MRSA, isolation precautions, training of hand hygiene, cleaning protocols, and decontamination of isolation ward. The HA-MRSA data and compliance of infection control measures between both groups were analyzed. . Through bundle interventions, the compliance with the isolation of MRSA raised from 55.88% to 92.86% and hand hygiene compliance increased from 90.07% to 93.23% ( < 0.05). The HA infection decreased from 1.87% to 1.71% ( > 0.05) and HA detection rate of MRSA declined from 2.63‰ to 1.00‰, respectively ( < 0.05). Multifaceted interventions can effectively prevent MRSA infection and transmission; this includes active surveillance, isolation precautions, increased hand hygiene compliance, environmental cleaning, and decontamination.
评价集束干预对新生儿重症监护病房(NICU)获得性耐甲氧西林金黄色葡萄球菌(MRSA)感染的疗效。本研究纳入了 2014 年 1 月至 2017 年 2 月在中国厦门 NICU 接受治疗的 11277 例婴儿,回顾性分析了患者的人口统计学和临床资料。2014 年至 2015 年的患者作为对照组,2016 年至 2017 年的患者为实验组。实施集束干预措施,包括筛查 MRSA、隔离预防措施、手卫生培训、清洁方案和隔离病房消毒。分析两组患者的 HA-MRSA 数据和感染控制措施的依从性。通过集束干预,MRSA 隔离的依从性从 55.88%提高到 92.86%,手卫生依从性从 90.07%提高到 93.23%(<0.05)。HA 感染率从 1.87%降至 1.71%(>0.05),HA 检测的 MRSA 检出率从 2.63‰降至 1.00‰(<0.05)。多方位干预可有效预防 MRSA 感染和传播,包括主动监测、隔离预防措施、提高手卫生依从性、环境清洁和消毒。
