In recent years, interscalene brachial plexus block (ISB) and selective supraclavicular nerve block (SNB) have been associated with successful outcomes in the treatment of fractures of the clavicle while avoiding complications. Description of the case: A 67-year-old male sustained a fracture of his right clavicle following a fall. Regional anesthesia was favored for his treatment after obtaining informed consent as he would have been exposed to high risk if treated under general anesthesia due to his medical history of coronary artery disease and chronic obstructive pulmonary disease. Ultrasound-guided combined application of ISB and selective SNB were utilized with a low-dose local anesthetic without the occurrence of any intraoperative or postoperative complication.
Low-dose ISB and selective SNB can be effectively and safely used for clavicular fracture treatment procedures, especially in patients who have a low pulmonary reserve. HIPPOKRATIA 2019, 23(1): 25-27.
近年来,肌间沟臂丛神经阻滞(ISB)和选择性锁骨上神经阻滞(SNB)在治疗锁骨骨折方面取得了成功的结果,同时避免了并发症。病例描述:一名67岁男性跌倒后右侧锁骨骨折。由于他有冠状动脉疾病和慢性阻塞性肺疾病病史,若接受全身麻醉治疗会面临高风险,因此在获得知情同意后,选择区域麻醉进行治疗。采用超声引导联合应用ISB和选择性SNB,并使用低剂量局部麻醉药,未发生任何术中或术后并发症。
低剂量ISB和选择性SNB可有效、安全地用于锁骨骨折治疗手术,尤其是肺储备功能差的患者。《希波克拉底》2019年,第23卷第1期:25 - 27页。
