Abstract

The aim of this study is to investigate, by a validated in vitro model, the effect of diabetic plasma on ejaculated human spermatozoa. Plasma of 51 male diabetic patients (mean age 62.28 ± 9.28 years) was selected according to their HbA level: low HBA  ≤ 5% (31 mmol/mol), moderate HBA 6%-8% (42-64 mmol/mol) and high HBA  ≥ 10% (86 mmol/mol). The plasma was tested on eighteen normal semen samples by analysing gametes motility using a computer Sperm Class Analyzer® and their corresponding oxidative stress (OS) status using thiobarbituric acid-reactive substances assay. The results indicated that diabetic plasma affected all sperm motility parameters with high HbA showing the most important deleterious effects. Low gametes' straight-line velocity was observed in high HbA level, mainly after 20 min of co-incubation (8.78 ± 0.47 µm/s). Also, the highest lipid peroxidation (nmoles MDA/10 SPZ) was observed in high HbA values (0.92 ± 0.09), higher than those in spermatozoa treated with H O (0.85 ± 0.04). Conclusively, a direct impact of diabetic plasma on spermatozoa is revealed with overexpression of OS as the underlying mechanism. These findings suggested that it is strongly recommended to control clinically the glycaemic level and OS in diabetic patients for the maintenance of male fertility.

本研究的目的是通过一个经过验证的体外模型,研究糖尿病血浆对射出的人类精子的影响。根据糖化血红蛋白(HbA)水平,选取了51名男性糖尿病患者(平均年龄62.28±9.28岁)的血浆:低HbA≤5%(31 mmol/mol)、中度HbA 6%-8%(42-64 mmol/mol)和高HbA≥10%(86 mmol/mol)。通过使用计算机精子分类分析仪分析配子活力以及使用硫代巴比妥酸反应性物质测定法检测其相应的氧化应激(OS)状态,对18份正常精液样本进行了该血浆测试。结果表明,糖尿病血浆影响了所有精子活力参数,高HbA显示出最重要的有害影响。在高HbA水平下观察到配子直线速度较低,主要是在共同孵育20分钟后(8.78±0.47 µm/s)。此外,在高HbA值样本中观察到最高的脂质过氧化水平(每10个精子中的丙二醛纳摩尔数)(0.92±0.09),高于用H₂O₂处理的精子(0.85±0.04)。总之,揭示了糖尿病血浆对精子有直接影响,其潜在机制是氧化应激的过度表达。这些发现表明,强烈建议临床上控制糖尿病患者的血糖水平和氧化应激,以维持男性生育能力。
