Abstract

BACKGROUND

Diabetic foot ulcer (DFU) is one of the most terrifying diabetic complications for patients, due to the high mortality rate and risk for amputation. During the COVID-19 pandemic, many diabetic patients limited their visits to the hospital, resulting in delays for treatment especially in emergency cases.

OBJECTIVE

This study aimed to compare the characteristics of patients with DFU pre- and during COVID-19 pandemic period. Methods: This study was a retrospective cohort study using foot registry data. We compared our patients' characteristics pre-COVID-19 pandemic period (1 March 2019-28 February 2020) and during COVID-19 pandemic period (1 March 2020-28 February 2021).

RESULTS

Cohorts of 84 and 71 patients with DFU pre- and during COVID-19 pandemic period, respectively, were included in this study. High infection grade (66.7% vs 83.1%,  = .032), osteomyelitis event (72.6% vs 87.3%,  = .04), leukocyte count (15 565.0/μL vs 20 280.0/μL,  = .002), neutrophil-to-lymphocyte ratio (7.7 vs 12.1,  = .008), waiting time-to-surgery (39.0 h vs 78.5 h,  = .034), and number of major amputation (20.2% vs 39.4%,  = .014) were significantly higher during the COVID-19 pandemic period.

CONCLUSION

During the COVID-19 pandemic, patients with DFU had more severe infection, higher proportion of osteomyelitis, longer waiting time for getting surgical intervention, and higher incidence of major amputation.

糖尿病足溃疡(DFU)是糖尿病患者最可怕的并发症之一,因为其死亡率和截肢风险较高。在 COVID-19 大流行期间,许多糖尿病患者限制了去医院就诊的次数,导致治疗延误,尤其是在紧急情况下。

目的

本研究旨在比较 DFU 在 COVID-19 大流行前后的患者特征。

方法

本研究为回顾性队列研究,使用足部登记数据。我们比较了 COVID-19 大流行前(2019 年 3 月 1 日至 2020 年 2 月 28 日)和 COVID-19 大流行期间(2020 年 3 月 1 日至 2021 年 2 月 28 日)的患者特征。

结果

本研究分别纳入了 COVID-19 大流行前和大流行期间 DFU 患者的 84 例和 71 例。大流行期间,高感染等级(66.7% vs 83.1%,=0.032)、骨髓炎事件(72.6% vs 87.3%,=0.04)、白细胞计数(15565.0/μL vs 20280.0/μL,=0.002)、中性粒细胞与淋巴细胞比值(7.7 vs 12.1,=0.008)、手术前等待时间(39.0 h vs 78.5 h,=0.034)和主要截肢率(20.2% vs 39.4%,=0.014)均显著更高。

结论

在 COVID-19 大流行期间,DFU 患者的感染更严重,骨髓炎的比例更高,接受手术干预的等待时间更长,且主要截肢的发生率更高。
