To analyze the characteristics of trigeminal event-related potentials (tERPs) in different kinds of olfactory disorders (OD), and to evaluate the importance of tERPs for the patients with olfactory dysfunction. Clinical data of 314 patients with olfactory dysfunction from the Smell and Taste Clinics in Beijing Anzhen Hospital from 2015 to 2021 were retrospectively reviewed, including 158 males and 156 females, aging from 6 to 78 years. The control group consisted of healthy people from medical examination center, who were gender and age matched. The clinical characteristics of OD were analyzed using Sniffin' Sticks test, olfactory event-related potentials (oERPs), tERPs and acoustic rhinometry test. SPSS 17.0 software was used to compare the difference of tERPs between the two groups, and to analyze the related factors affecting trigeminal function. The ratio of tERPs presence was different in OD caused by different reasons: head traumatic OD (54.9%), post-virus infection OD (63.6%), sinonasal inflammatory OD (68.4%) and OD due to other causes (56.9%). Compared with controls, tERPs signals in OD patients showed a significant lower amplitude in the N wave (all <0.001), and lower amplitude in the P wave in most OD patients (head trauma =-4.11, <0.001; sinonasal inflammation =-2.04, 0.046; others =-2.40, =0.020) except in OD by post-virus infection (=-1.98, =0.052). tERPs signals in OD patients by sinonasal inflammation showed longer latency in the N wave (=2.15, =0.036), but this difference was not observed in other OD patients (all >0.05). tERPs signals were significantly correlated with the Sniffin' Sticks score, deficiency of oERPs and nasal minimum cross-sectional area (all <0.05). OD patients show neurophysiologic deficits in trigeminal function. The absence of tERPs or lower amplitude in N waves are the important characteristics of patients with OD.
分析不同类型嗅觉障碍(OD)中三叉神经事件相关电位(tERPs)的特征,并评估tERPs对嗅觉功能障碍患者的重要性。回顾性分析2015年至2021年北京安贞医院嗅觉与味觉门诊314例嗅觉功能障碍患者的临床资料,其中男性158例,女性156例,年龄6至78岁。对照组为来自体检中心的健康人,在性别和年龄上进行匹配。采用嗅觉棒测试、嗅觉事件相关电位(oERPs)、tERPs和声反射鼻测量测试分析OD的临床特征。使用SPSS 17.0软件比较两组tERPs的差异,并分析影响三叉神经功能的相关因素。不同原因导致的OD中tERPs出现率不同:头部创伤性OD(54.9%)、病毒感染后OD(63.6%)、鼻-鼻窦炎性OD(68.4%)和其他原因导致的OD(56.9%)。与对照组相比,OD患者tERPs信号的N波振幅显著降低(均<0.001),大多数OD患者的P波振幅降低(头部创伤=-4.11,<0.001;鼻-鼻窦炎=-2.04,0.046;其他=-2.40,=0.020),病毒感染后OD除外(=-1.98,=0.052)。鼻-鼻窦炎性OD患者的tERPs信号N波潜伏期延长(=2.15,=0.036),但其他OD患者未观察到这种差异(均>0.05)。tERPs信号与嗅觉棒评分、oERPs缺失和鼻腔最小横截面积显著相关(均<0.05)。OD患者存在三叉神经功能的神经生理学缺陷。tERPs缺失或N波振幅降低是OD患者的重要特征。
