Abstract

BACKGROUND

Despite the recent improvements in the surgical treatment of congenital kyphosis, this surgery may be associated with high rate of complications "particularly when patients are symptomatic preoperatively".

MATERIALS AND METHODS

Medical profiles of 40 patients with symptomatic congenital kyphosis were retrospectively reviewed. Perioperative complications were recorded and divided in two groups including catastrophic complications (neurologic deficit, pulmonary thromboembolic events, and death) and major complications (infection, deep vein thrombosis, device failure, and dural injury).

RESULTS

Catastrophic surgical complications occurred in nine (22.5%) patients including seven neurological deficits and two death. A significant association was observed between the incidence of major complications and type III of congenital kyphosis (P = 0.021). Major complications occurred in 14 (30%) patients.

CONCLUSION

Surgical treatment significantly improve symptomatic congenital kyphosis deformity; however, surgery of symptomatic patient may be associated with higher rate of complications and even death.

尽管先天性脊柱后凸的外科治疗近期有所改善,但该手术可能伴有较高的并发症发生率,“尤其是术前有症状的患者”。

材料与方法

回顾性分析40例有症状的先天性脊柱后凸患者的病历资料。记录围手术期并发症,并分为两组,包括灾难性并发症(神经功能缺损、肺血栓栓塞事件和死亡)和主要并发症(感染、深静脉血栓形成、器械故障和硬脊膜损伤)。

结果

9例(22.5%)患者发生灾难性手术并发症,包括7例神经功能缺损和2例死亡。主要并发症的发生率与先天性脊柱后凸III型之间存在显著相关性(P = 0.021)。14例(30%)患者发生主要并发症。

结论

手术治疗可显著改善有症状的先天性脊柱后凸畸形;然而,有症状患者的手术可能伴有更高的并发症发生率甚至死亡。
