[Causes of transfusion incidents].

Even though the field of transfusion medicine is thoroughly researched, there still occur fatal transfusion accidents which are mostly due to organizational lacks. The error possibilities were divided in four groups, corresponding to the stages of a complete transfusion. A transfusion protocol is proposed.
