Managing soft tissue defects in the heel presents a significant challenge for surgeons due to the heel's crucial role in weight-bearing. A free instep flap with end-to-side nerve coaptation offers an effective solution for reconstructing large heel defects. Four patients were referred to our hospital with soft tissue lesions in the weight-bearing heel and instep area. Their heels were reconstructed using a free instep flap harvested from the opposite foot. In three cases, an end-to-side arterial anastomosis was performed. In the fourth case, due to a midfoot amputation, an end-to-end arterial anastomosis was performed more distally to preserve collateral branches. In all cases, the vein was anastomosed end-to-end to the posterior tibial vein. The epineurium of the medial plantar nerve was opened, and an end-to-side nerve repair was performed. Our patients demonstrated satisfactory sensation, and none experienced flap shearing or ulceration. The flap thickness and shape were ideal, closely resembling the contralateral normal side.
由于足跟在负重过程中起着关键作用,因此处理足跟部软组织缺损对外科医生来说是一项重大挑战。带端侧神经吻合的游离足背皮瓣为重建大面积足跟缺损提供了一种有效的解决方案。4例足跟负重区和足背软组织病变患者被转诊至我院。采用对侧足部的游离足背皮瓣对其足跟进行重建。3例采用端侧动脉吻合。第4例因中足截肢,在更远端进行端端动脉吻合以保留侧支。所有病例均将静脉与胫后静脉端端吻合。切开足底内侧神经的神经外膜,进行端侧神经修复。我们的患者感觉良好,均未出现皮瓣剪切或溃疡。皮瓣厚度和形状理想,与对侧正常侧非常相似。
