This study is the first to demonstrate the feasibility of at-home adherence monitoring using a point-of-care urine assay for tenofovir (TFV). Undetectable TFV is strongly associated with lower viral suppression (odds ratio, 0.19, 95% CI, 0.10-0.34; < .001), supporting a streamlined, effective approach for monitoring adherence between viral load assessments.
