Abstract

Transcatheter electrosurgical septotomy aids the adequate expansion of fenestrated or branched endografts in patients with a postdissection thoracoabdominal aortic aneurysm and a narrow true lumen. Still, the intraprocedural identification of preexisting reentry tears in the septum can be challenging and time consuming. We report four cases of post-dissection thoracoabdominal aortic aneurysms endovascular treatment, where computed tomography angiography fusion imaging was used during transcatheter electrosurgical septotomy in association with intravascular ultrasound examination, to improve precision in septum tear cannulation and favor procedural length and radiation exposure reduction.

经导管电外科隔膜切开术有助于在患有夹层胸腹主动脉瘤且真腔狭窄的患者中充分扩张开窗或分支型覆膜支架。然而,术中识别隔膜中预先存在的再入撕裂可能具有挑战性且耗时。我们报告了4例夹层胸腹主动脉瘤的血管内治疗病例,在经导管电外科隔膜切开术中,计算机断层扫描血管造影融合成像与血管内超声检查联合使用,以提高隔膜撕裂插管的精度,并有助于缩短手术时间和减少辐射暴露。
