45 patients suffering from supra-diacondylar fractures of the distal end of the humerus were treated surgically and had a follow-up covering a time of 6 to 170 months. In 76% the results obtained were excellent. This demonstrates the superiority of operative treatment in this type of fracture. The most important factor for good exposure of the joint is osteotomy of the olecranon. Fixation of the fragments was carried out according to the principles of AO. The aim--stable fixation--could not be attained in all cases.
