Patient transfer form provides continuity of care.

General Hospital of Everett (WA) took the initiative in developing a new patient transfer form that has been adopted for statewide use. By using suggestions from a wide variety of sources, a hospital committee, which consisted of professionals from local extended care facilities and nursing homes, as well as from the hospital's staff, was able to develop a useful and effective patient transfer system.
