Thirty patients with cineangiographically confirmed mitral valve prolapse (MVP) were studied by radionuclide angiography at rest and during exercise. Twelve patients had isolated MVP and 18 patients had associated coronary artery disease. All patients had normal resting left ventricular ejection fraction and wall motion. Of 18 patients with MVP and coronary artery disease, the left ventricular ejection fraction remained unchanged during exercise in 15 patients, decreased in two patients, and increased in one patient. Of 12 patients with MVP without associated coronary artery disease, ejection fraction increased during exercise in 6 patients, remained unchanged in 4 patients, and decreased in 2 patients. We conclude that an abnormal left ventricular function response during exercise does not always indicate the presence of coronary artery disease and may occur in patients with isolated MVP. Furthermore,the left ventricular dysfunction of patients with isolated MVP may be obvious only during exercise.
对30例经心血管造影证实为二尖瓣脱垂(MVP)的患者在静息和运动时进行了放射性核素血管造影研究。12例患者为单纯MVP,18例患者合并冠状动脉疾病。所有患者静息时左心室射血分数和壁运动均正常。在18例合并MVP和冠状动脉疾病的患者中,15例患者运动时左心室射血分数保持不变,2例患者降低,1例患者升高。在12例无相关冠状动脉疾病的MVP患者中,6例患者运动时射血分数升高,4例患者保持不变,2例患者降低。我们得出结论,运动时左心室功能异常反应并不总是表明存在冠状动脉疾病,也可能发生在单纯MVP患者中。此外,单纯MVP患者的左心室功能障碍可能仅在运动时才明显。
