A case report concerning paroxysmal nocturnal hemoglobinuria is presented. The role of hemorheologic factors in perfusion of the placenta is clearly demonstrated. Fetal monitoring and the application of hypervolemic hemodilution enabled prolongation of the pregnancy until a healthy neonate could be delivered by Ceasarean section at the 34th week of gestation.
