Intraoperative monitoring is very important for protecting nerve roots during lumbar surgery for spinal degeneration. Our objective was to evaluate the correlation between the type of mechanical irritation and waveform by mechanically elicited EMGs during the surgery.
Mechanically elicited EMGs were recorded bilaterally on muscle groups innervated by the lumbar nerve roots in the area of surgery in 24 consecutive patients with lumbar spinal degenerative disease.
It was confirmed that surgical irritation produced 3 types of waveforms as discharges from nerve roots; short, waning and continuous discharges. Each waveform was easily elicited mechanically and was sensitively related to the type and strength of mechanical stimulation.
It was indicated that the mechanical irritation on the root could elicit the nerve root discharge with no post-operative nerve root deficit. It may be useful to monitor the mechanically elicited EMGs during the surgery for spinal degeneration.
术中监测对于腰椎退变手术中保护神经根非常重要。我们的目的是评估手术期间机械诱发肌电图中机械刺激类型与波形之间的相关性。
连续24例腰椎退行性疾病患者在手术区域由腰神经根支配的肌肉群双侧记录机械诱发肌电图。
证实手术刺激产生了3种类型的波形作为神经根放电;短时间、逐渐减弱和持续放电。每种波形很容易通过机械方式诱发,并且与机械刺激的类型和强度敏感相关。
表明对神经根的机械刺激可诱发神经根放电而无术后神经根功能缺损。在腰椎退变手术期间监测机械诱发肌电图可能是有用的。
