An 11-year-old girl had many features of the nevoid basal cell carcinoma syndrome. Extensive cysts of the maxilla and mandible were treated with marsupialization and secondary enucleation that resulted in a gradual but significant response. Long-term observation will be necessary to detect the possible development of either basal cell carcinomas or the recurrence of odontogenic keratocysts.
