We report on a 30-year old female with bilateral visual loss within four days. She assured that she had been able to do computer-work until four days before presentation. Ophthalmoscopy and echography established the diagnosis of a bilateral papilledema. Visual acuity was 20/500 in both eyes. Perimetry revealed a small central residual visual field. Further examinations lead to the diagnosis of a primary pseudotumor cerebri.
Conservative treatment of the elevated intracranial pressure and ventriculo-peritoneal shunting improved visual acuity and visual field and lead to a decrease of the papilledema. Follow up time was 30 months.
In a small number of patients pseudotumor cerebri presents with an acute visual loss. Immediate diagnosis and therapy can lead to an improvement of vision.
我们报告一名30岁女性，在四天内出现双侧视力丧失。她保证在就诊前四天还能从事电脑工作。眼底镜检查和眼部超声检查确诊为双侧视乳头水肿。双眼视力均为20/500。视野检查显示中央有一小片残余视野。进一步检查诊断为原发性假性脑瘤。
对颅内压升高进行保守治疗及脑室-腹腔分流术改善了视力和视野，并使视乳头水肿减轻。随访时间为30个月。
少数假性脑瘤患者会出现急性视力丧失。及时诊断和治疗可使视力得到改善。
