Augmentation of the depressed nasal dorsum may be accomplished in a variety of ways by utilizing a number of different implant materials. Several techniques are discussed and illustrated. Small areas of depression may be corrected with viable or nonviable cartilage or synthetic implants through an intercartilaginous incision. More extensive saddling is best augmented using either iliac bone placed subperiosteally or irradiated cartilage. When the implant is sizable or a two piece "L shaped" strut is required, placement is done through a gingivolabial sulcus incision.
