Abstract

BACKGROUND/AIMS: Only a small fraction of patients with chronic hepatitis C have a sustained biochemical or virologic response to a standard course of alpha-interferon therapy. Thus, alternative treatments are needed particularly for non-responders. The main objective of this study was to evaluate the efficacy of alpha-interferon in combination with ribavirin in patients who had not responded to a previous course of alpha-interferon.

METHODOLOGY

In this prospective open trial, 26 patients who had not responded to a previous course of interferon monotherapy, were treated for 6 months with a combination of alpha-interferon 2b, 5 MU thrice weekly, plus ribavirin 1000 or 1200 mg daily. They were followed-up for at least 6 months after therapy.

RESULTS

At the end of treatment, 3 patients (12%) had normal aminotransferase levels and two (8%) tested negative for HCV-RNA in serum. After 6 months of follow-up, all patients had HCV viremia and only one (3.8%) was still in biochemical remission. One patient dropped out because of side effects and another was lost during follow-up.

CONCLUSIONS

alpha-interferon-ribavirin combination is ineffective in treating patients who had had no response to interferon monotherapy.

背景/目的:在慢性丙型肝炎患者中,只有一小部分人对标准疗程的α干扰素治疗有持续的生化或病毒学应答。因此,尤其对于无应答者,需要其他治疗方法。本研究的主要目的是评估α干扰素联合利巴韦林对既往α干扰素疗程无应答患者的疗效。

方法

在这项前瞻性开放试验中,26例对既往干扰素单一疗法无应答的患者,接受了为期6个月的α干扰素2b联合治疗,每周三次,每次5 MU,加用利巴韦林,每日1000或1200 mg。治疗后对他们进行了至少6个月的随访。

结果

治疗结束时,3例患者(12%)转氨酶水平正常,2例(8%)血清HCV-RNA检测呈阴性。随访6个月后,所有患者均有HCV病毒血症,只有1例(3.8%)仍处于生化缓解状态。1例患者因副作用退出,另1例在随访期间失访。

结论

α干扰素-利巴韦林联合治疗对干扰素单一疗法无应答的患者无效。
