To investigate the possible role of apoptosis in the development of uterine cervical carcinoma.
Formalin-fixed, paraffin-embedded samples from 190 patients [41 patients with severe dysplasia (SD); 37 with carcinoma in situ(CIS); 31 with microinvasive carcinoma (MIC), 40 with fran invasive large cell non-keratinizing epidermoid carcinoma (IC)], and 41 samples from normal cervical squamous epithelium (NE) were studied. The number of apoptotic cells was assessed in situ by the TDT-mediated dUTP-biotin nick end labeling (TUNEL) method. PCNA, p53 and bcl-2 were demonstrated immunohistochemically.
(1) In NE, TUNEL-positive cells were found in the superficial layer and PCNA-positive cells were confined in the lamina profunda, while in cervical neoplasia these cells were irregulasly scattered throughout the cervical lesions. (2) The TUNEL staining index decreased while PCNA increased with progression of the neoplasm, showing a significant negative correlation between apoptosis and proliferation. (3) In patients with SD and CIS who had overexpression of p53 and bcl-2 proteins, the cells positively stained by TUNEL were significantly less in number than in these with negative p53 and bcl-2 expression. No such observation for PCNA expression.
These results suggest that apoptosis is associated with the early process of cervical carcinogenesis and apoptosis is closely correlated with overexpression of p53 and bcl-2 proteins.
探讨细胞凋亡在子宫颈癌发生发展中的可能作用。
研究了190例患者福尔马林固定、石蜡包埋的样本[41例重度不典型增生(SD)患者;37例原位癌(CIS)患者;31例微浸润癌(MIC)患者,40例浸润性大细胞非角化性表皮样癌(IC)患者],以及41例正常宫颈鳞状上皮(NE)样本。采用末端脱氧核苷酸转移酶介导的dUTP生物素缺口末端标记法(TUNEL法)原位评估凋亡细胞数量。免疫组织化学法检测增殖细胞核抗原(PCNA)、p53和bcl-2。
(1)在NE中,TUNEL阳性细胞见于表层,PCNA阳性细胞局限于深层,而在宫颈肿瘤中,这些细胞不规则地散布于宫颈病变处。(2)随着肿瘤进展,TUNEL染色指数降低而PCNA升高,显示凋亡与增殖之间存在显著负相关。(3)在p53和bcl-2蛋白过表达的SD和CIS患者中,TUNEL阳性染色的细胞数量明显少于p53和bcl-2表达阴性的患者。PCNA表达未见此现象。
这些结果表明,细胞凋亡与子宫颈癌发生的早期过程相关,且细胞凋亡与p53和bcl-2蛋白的过表达密切相关。
