Obstructive hydrocephalus caused by large or giant aneurysm is rare. In most reported cases, the aneurysm was located at the bifurcation of the basilar artery and direct surgical treatment was considered to be hazardous. Endovascular treatment of the aneurysm with a detachable two-lumen balloon-catheter was performed immediately after ventricular shunt placement.
