Pharmacy-nursing communications in an intravenous admixture program.

A centralized pharmacy admixture program is discussed, with emphasis on the forms used to communicate between pharmacists and nurses. Aspects of the programs described include the patient intravenous profile folder, preparation and delivery of i.v. solutions, preparation of piggyback solutions and prefilled syringes of reconstituted antibiotics, and emergency intravenous solution stock.
