Demonstration of a lucent tubular shadow projected over the liver on plain films of the abdomen would suggest air in the common hepatic and bile ducts. Occasionally, however, the normal periductal fat which surrounds and follows these ducts like a sleeve can simulate pneumobillia. Intravenous cholangiography should confirm the true nature of the entity.
